Membership
When you become a member of the Ohio Independent Provider Network, you are asked to
fill out a form that requests your personal information. Your information is used solely to
process your membership, donation, or request to be listed on the site. This information will
never be sold, rented, or traded to others for purposes other than providing you member
benefit opportunities.
By becoming a member, of the Ohio Independent Provider Network and /or by using this
website, you consent to the collection and use of the information you provide to us as
described above.

The Ohio Independent Provider Network website does NOT endorse or recommend any
Independent Provider Nurses who may list their service or availability on this website.
All Ohio Independent Providers are required to have an active provider number and an
annual criminal background check. Consumers are encouraged to ask the provider to bring
their annual criminal background check, nursing license, photo I.D. and CPR card to the
interview for your review. Prior to interviewing the provider, the consumer is encouraged to
ask the nurse provider for their nursing license number, which can be verified with the Ohio
Board of Nursing: http://www.nursing.ohio.gov/Verification.stm You can also use the
nursing verification site by simply typing in the provider’s last name and county he/she lives
in, to verify their nursing license is current and unencumbered. The site will provide you
with such information as whether or not, the nurse has been disciplined or has a pending
case or discipline. If you do not have access to a computer, call (614) 466-3947 for Ohio
nursing license verification.
